The Municipal Courts on behalf of the U.S. Department of State at present accepts passport applications for U.S. citizens wanting to get a new passport. Citizens may submit an Application for U.Southward. Passport (DS-11) in person.  The passport renewal application is not accepted at the Municipal Court locations, even so instructions on completing and mailing a passport renewal awarding can be found on the U.S. Department of Country website world wide web.travel.country.gov.

You must apply in person using form DS-11 if at least one of the following is true:

• You are applying for your first U.S. passport, or
• You are under the age of 16, or
• Your previous U.South. passport was issued when you were under historic period 16, or
• Your previous U.Southward. Passport was lost, stolen, or damaged, or
• Your previous U.S. Passport was issued more than 15 years ago, or
• Your name inverse since your U.Southward. passport was issued and you are unable to legally document your name change.

What will you need?

A completed DS-xi awarding

• Download awarding online at travel.state.gov
• Consummate in black ink only
• Practice not sign application until requested to do so by a passport credence agent

Notation: Passport renewal application DS-82 is not accepted at the Municipal Court acceptance locations, however instructions on completing and mailing a DS-82 passport renewal application can exist found on the U.South. Section of Land website world wide web.travel.state.gov.

Proof of Identity

Bring one original (plus one photocopy forepart and back) of the following:

• Valid Driver's License
• Valid Country Identification Card
• Naturalization Certificate
• Prior Undamaged U.S. Passport (may exist expired)
• Government Employee ID
• Official Military ID Card
• For minors: Under 16 years of historic period both parents must be present and provide official consent. If both parents cannot be nowadays, go to www.travel.country.gov for more information.

Proof of U.S. Citizenship

Bring one original (plus one photocopy) of the following:

• Previously issued U.S. Passport undamaged (may exist expired)
• U.Southward. Birth Certificate or certified re-create issued past the city, county, or land
• Certificate of Naturalization
• Consular Study of Birth Away or Certification of Nativity
• Certificate of Citizenship
• For minors under 18, the original nascence certificate must be presented every fourth dimension an application is submitted.
• Minors with a previous passport must submit the original or a copy of their birth certificate along with the expired or current valid passport.

Note: Original proof of citizenship must be submitted with your application, however, the original document volition be returned to you by postal service in a separate envelope.

Passport Photo (pictures are non provided on site)

• (1) one color photo
• 2"x2" inches in size
• Taken inside the past vi months, showing current appearance
• Total Face, with neutral facial expression
• Forepart view with a plain white or fair groundwork
• Between 1" inch and one 3/eight" inches from the bottom of the chin to the pinnacle of the head
• Taken in normal street attire. Camouflage and uniform clothes is prohibited on a passport photo
• No spectacles

PAYMENTS

2 (2) payments are needed to process your passport application. One payment is for the application fee and the second payment is for the credence fee, meet beneath for more payment data.

Where tin you apply?
The Municipal Courts has 2 Passport Acceptance locations, one in Key Houston at the Herbert West. Gee Municipal Courthouse and one in Due west Houston at the Westside Command Station.  Appointments are xxx minutes in duration and can be fabricated 14 days in advance.  A separate appointment is needed for every applicant that will be applying for a U.South. passport.  As a courtesy, please arrive fifteen minutes before your scheduled engagement.

Check the status of your application
Check the status of your U.S. Passport application in vii-10 business days by going to https://passportstatus.state.gov, in that location you may register your email address to receive automatic e-mail updates. You will need the applicant's Last Name, Date of Nascency, and Social Security Number (Last 4 digits).
